Cashless claim rejected asked to pay

Hospital rejected cashless claim approval. Now I have to pay 5 lakhs from pocket. What are my options?

You can approach the Insurance Ombudsman if the company is not responding to your grievance for 30 days. (Reference: 7633)

Did you try contacting Ditto Insurance? They help with claims sometimes.
